MEMBERSHIP TERMS AND CONDITIONS – Cherish Card - Senior Citizen Privilege Card

“Cherish Card” refers to the member-exclusive service provided by Sunway Healthcare Group (hereafter called SHG) for Cherish Card holders. The Cherish Card is a privilege card which gives Card Holders the benefits of enjoying discounts, privileges, events from time to time offered by SHG (referred collectively as Benefits). Card Holder means all the holders of a valid Cherish Card. This AGREEMENT is made and agreed to between Card Holder (You) and Sunway Medical Centre Sdn Bhd.

1. The SMCD Cherish Card is a privileges card which gives Card Holders the benefits of enjoying discounts, privileges, and events from time to time offered by SunMed Velocity.
2. The SMCD Cherish Card is non-transferable and non-assignable and is only for the exclusive use of the Card Holder only.
3. The SMCD Cherish Card is NOT a point accumulation card. It does not allow or require Card Holder to collect points for gift exchange or similar purposes.
4. The SMCD Cherish Card is NOT a credit card, debit card or the like
5. Discount privileges are valid only upon presentation of your SMCD Cherish Card during registration. No reimbursement or refund on previous or back-dated transactions if card holders fail to present theSMCD Cherish Card during registration.
6. Discount privileges strictly apply to self-pay cases only.
7. Unless otherwise stated, the card cannot be used in conjunction with other cards and/ or other promotions.
8. SMCD reserves the right to discontinue, modify or add any terms and conditions, benefits or privileges granted by this card as deemed necessary without prior notice.
9. SMCD is not liable for any loss or damage incurred by the Card Holders due to any unathorized use of the SunMed Velocity Cherish Card.
10. Any request for replacement of SMCD Cherish Card for any reason whatsoever including but not limited to loss or damaged cards, shall be subjected to a fee of RM10 per card.
11. Should any of the conditions and privileges are misused or abused, SMCD is entitled to suspend and terminate the Card Holder’s right to use the SMCD Cherish Card without prior notification.
12. SMCD undertakes that all personal information supplied by the Card Holder for the registration of the SMCD Cherish Card shall not be disclosed to any third party unless with the prior written consent of the Card Holders or required by law
13. The SMCD Cherish Card is a life time membership programme without a need of renewal.
14. The SMCD Cherish Card is applicable for Malaysian and expatriates aged 60 years and above.
15. The discounts offered for SunMed@Home are only valid on non-packages services. It is also valid on medical supply and mileage charges.
16. The discount from Sunway TCM Centre is not applicable for consultation, medication and consumables items.
